Taking forms to a higher level requires the use of Javascript to keep track of checkbox and radio state, and to make calculations based on size & position.  Currently, my favorite implementation of this technique is Niceforms v.2.0 © Lucian Slatineanu - EmblematiqNiceforms v.2.0 © Lucian Slatineanu - Emblematiq.

Tough & Reliable, ...with a no-stick surface.  If you want it to last, do it right.  (X)

Loud and obnoxious style.  Let this be a tribute to all those who fight our California wildfires.  (X)

Nice warm Earth-tones, ...ahhhh.  Promotes a sence of peace and security.  (X)

A theme to remind us of great things in nature.  Reminds me of early morning mountains in the Northwest.  (X)
